What's on the blood pressure log sheet

Each sheet has room for 22 readings on US Letter and 24 on A4, with these columns:

At the top there is space for your name, your monitor's brand and model, and a page number. The sheet has no target numbers and no color-coded ranges on purpose: it is a blank log, and what your readings mean is a question for your doctor.

How to fill in your blood pressure log

  1. Print the sheet. Download the PDF in US Letter or A4 and print as many pages as you need. Write your name and your monitor's brand and model at the top.
  2. Write each reading as you take it. Fill in the date, time, systolic (top number), diastolic (bottom number) and pulse straight from your monitor's display.
  3. Add arm, position and notes. Note which arm you used, whether you were sitting, standing or lying down, and anything worth mentioning, such as medication taken or how you felt.
  4. Bring it to your appointment. Take the sheets with you so your doctor or nurse can review the readings with you.

Keeping several pages of readings

When a sheet is full, start a new one and fill in the page number at the top. Keep the pages in date order in a folder or on a clipboard next to your monitor, so the whole log is in one place when you need it.

Is the blood pressure log really free?

Yes. Both PDFs are free to download, print and copy. There is no sign-up and no email required.

Can I print it at home?

Yes. It is a single page in black and light gray with a little red, so it prints fine on any home printer, in color or black and white. Print as many copies as you need.

Should I choose US Letter or A4?

Choose US Letter if you are in the United States, Canada or Mexico. Choose A4 almost everywhere else, including the UK, Europe, India and Australia. If you are not sure, check the paper size setting in your printer dialog.

Can I share the log sheet with my doctor?

Yes. Bring the filled-in sheets to your appointment, or scan or photograph them and send them the way your doctor's office prefers.
