What's inside the blood pressure log book
The booklet is 14 pages, in black and light gray with a little red, so it prints fine in color or black and white:
- Cover page with lines for your name, your doctor, your doctor's phone number, your monitor's brand and model, and the start date.
- 12 weekly pages, one week per page. Each day has a morning row and an evening row, with columns for date, time, systolic (mmHg), diastolic (mmHg), pulse (bpm), arm (L or R) and notes.
- Questions for my doctor: a lined last page for questions you want to ask at your next appointment.
There are no target numbers, charts or color-coded ranges in the book. It is a blank log, and what your readings mean is a question for your doctor.
How to use your blood pressure journal
- Fill in the cover. Add your doctor's details and your monitor's brand and model, so everything is in one place.
- Write each reading down right away. Copy the numbers from your monitor's display before you forget them.
- Note the time and the arm. Use the morning or evening row for the day, write the exact time, and mark L or R.
- Use the Notes column for what your doctor asked you to track, such as when you took a medication or anything unusual that day.
- Bring the book to your appointment. Add questions to the last page as they come up.
When and how often to measure: follow the schedule your doctor or nurse gave you. For general guidance on measuring at home, see the American Heart Association's Monitoring your blood pressure at home.
Other formats: single record sheets and a spreadsheet
Need a single blood pressure record sheet instead of a booklet? Use the free printable blood pressure log, one page with 22 to 24 rows. If you prefer typing, the blood pressure log spreadsheet works in Excel and Google Sheets.
